If you have a valid Hong Kong Identity Card and medical proof from an ophthalmologist certifying your visual impairment, you are eligible to be a member of the centre. Application forms can be obtained by calling 3723 8325 or click here to download.

Please notify the centre staff immediately by phone and indicate in writing or Braille on the damaged item when returning it to the centre so that the staff could follow up accordingly.

All members are obligated to keep the loaned items in a proper condition to avoid artificial damage, e.g., be careful when drinking water to avoid getting the loaned item wet, keep the player clean or use head-cleaning fluid to wipe the magnetic head of the player so that the player could be more durable.

Compensation for audio tapes and CDs are counted by the number of cassettes or discs, while that of braille books are counted by the number of volumes. For borrowed videotapes, movie discs, DVDs, and music CDs, compensation will be based on the quantity and amount determined by the centre manager as sufficient to replace the item. In addition, an administrative fee of 10% of the item's price would be charged.

Members who have made changes of their personal information such as mailing address, contact phone number, name, etc., should notify the centre staff at 3723 8325 as soon as possible so that their personal records can be updated timely so as to avoid lost mail. All personal data is kept strictly confidential and will not be disclosed to any outside parties without prior consent.
